We recently granted $50,000 to Catholic Health Services of Long Island for their diabetes outreach program at St. Francis Hospital. The grant’s purpose is to deploy health coaches to help diabetes patients get more involved in their treatment. “In recent years, nearly 15,000 Long Islanders were hospitalized with diabetes,” says David M. Okorn, LICF’s executive director. “This program will enable patients to take a more active role in managing their health, particularly for people with chronic diseases, trained health coaches can get patients more involved in their care and thereby improve clinical outcomes.”
